Horizon Hospice is a team of qualified and licensed health professionals who provide skilled care to individuals at their residence as prescribed by their physician.
Horizon Hospice will provide RNs, social workers, home health aides, spiritual advisors (chaplains) and trained volunteers as needed by the unique and changing needs of the patient and their family. Hospice service also provides illness related medications, medical equipment and supplies.
Services are available from Horizon Hospice to anyone without regard to financial capabilities. Medicare Part A and many private insurance policies provide hospice care benefits.
