For 26 years, Horizon Hospice has cared for ALL of San Diego County from the coastal cities of Oceanside, Carlsbad, Encinitas and Del Mar, across the North County cities of Vista, San Marcos, Fallbrook and Escondido, out to the East County cities of La Mesa, El Cajon, Santee and Alpine, and south to National City, Chula Vista and Imperial Beach.
Centrally headquartered in Poway, Horizon is the only Medicare approved hospice providing hospice care to many of San Diego's more remote communities including Borrego Springs, Warner Springs, Ranchita, San Felipe, Lake Henshaw, Julian, Romona, Delzura, Descanzo, Jamul and Campo.
Horizon Hospice staff come to the patient wherever they reside. Entering hospice care means that many patients can live at home, while others choose to remain in residential care or skilled nursing environments. No matter the location, Horizon's team of professionals deliver an individualized regime of palliative care, including pain relief and symptom management. The team is also concerned with the emotional, social and spiritual conditions of the patient's caregivers, family and loved ones.
